To Obtain A Copy Of Your Medical Records:

For complete records, choose the Release of Patient Records – Complete; for partial records, choose Release of Patient Records-Partial.

Be sure to identify to whom the records are being released and purpose for the disclosure.

Complete, sign and submit the Release. Allow 10 business days to process your request.

If the patient is 18 years or older, s(he) must request and sign for the records or give us written permission for release to a parent or other individual.

Per Connecticut law, the charge is 45¢ per page for copies plus postage fee if the records are to be mailed.

For transfer to another practice, tell us if you want office notes only or all consult letters, lab and X-ray results.

If you plan to return to NEP after a finite period, we will keep your chart on file. Otherwise, transferred records are placed in storage one year after transfer and retained for seven years.
