CAMP & SCHOOL FORMS To efficiently manage the enormous volume of school and camp forms, we ask that you complete the following prior to submitting school, camp or sports forms:
- Patient information/demographics
- Allergies
- Chronic medical conditions
- Past surgery or hospitalization
- Name and dosage of each prescription or over-counter medication taken regularly
- Restrictions in diet or physical activity
 Form fees may be paid upon submission or pick-up: $10.00 per form per patient, or $20.00 for two or more forms when submitted simultaneously. |
