TRANSFER / NEW TO AREA PATIENTS  Before your first sick or well visit at New England Pediatrics, request that your child's medical records (especially vaccine records) be sent directly to our office or bring them with you to your first visit. The transfer of records may take several days to weeks, so please request these records at least three weeks prior to your visit. Your past pediatrician will supply you with the necessary HIPPA Authorization for Release of Records. Health records are also required before we can complete camp or school forms.
Please bring your insurance card and a photo ID along with the following completed forms:
Two important caveats for new patients: we cannot see new patients during our Stamford walk-in hours as these appointments do not allow for thorough new patient assessment and we cannot accept a new patient whose family does not permit routine childhood vaccinations.
