Allergy protocol, School and Camp form fee will Increase

(Effective March 1^st, 2025) to $15.00 or $30.00, for two or more forms when submitted simultaneously.

Payment is required at the time of submission

And will not be processed until payment has been received.

Expect a processing time of 7-10 business days.  All forms will be automatically made available on the patient portal.
