CAMP, SCHOOL,& ALLERGY PROTOCOL FORMS To efficiently manage the enormous volume of school, camp and allergy protocol forms, we ask that parents complete the following prior to submitting  forms:

• Patient information/demographics
• Allergies (include Allergist Protocol if applicable)
• Chronic medical conditions
• Past surgery or hospitalization
• Daily medications (name and dosage)
• Restrictions in diet or physical activity

Allergy protocol, school and camp form fees may be paid upon submission or pick-up: $10.00 per form per patient, or $20.00 for two or more forms when submitted simultaneously.
